- A size the body cannot safely hold in one session. How much fat survives is limited by the blood supply of the tissue, not by what is asked for
The pathway, step by step

Consultation with Dr Kpaduwa₦150,000
Thirty minutes with the surgeon herself, by video. You bring your goals, your history and photographs; she tells you what is involved, what she would and would not do, and whether you should be doing anything at all.

Examination and a written quote
An in-person examination in Lagos, then a written quote that says what is included and what is not. Costings come from the team in writing, not from the surgeon in the room.

Screening before a date is offered
Nicotine, medicines, blood pressure, and whether your weight is still changing all affect safety and timing. The clinical team reads your answers before anyone offers you a date.

The operation
Under general anaesthetic. Liposuction first, then the fat is purified and placed in the layer under the skin, above the buttock muscle and never into it. That placement rule is the single biggest safety advance in this operation.

A compression garment, written instructions on sitting and sleeping, and named follow-up appointments.
Recovery, realistically
Week 1
Swelling and bruising, usually sorest where the fat was taken. Walking from the first day to keep the blood moving. No sitting directly on the buttocks.
Weeks 2 to 3
Most people with desk work are back at it around two weeks, following the sitting instructions they were given.
Weeks 3 to 6
The garment continues. Gentle exercise returns as the surgeon allows.
Months 3 to 6
Some of the transferred fat does not survive and is reabsorbed. The shape you see at about six months is the result.
Risks you should be told about
- Fat embolism, where fat enters a large vein in the buttock. Rare and serious, and the reason BBL has the reputation it does. Placing fat only above the muscle is the technique that reduces it
- Loss of part of the transferred fat, so the result is smaller than it looks at the end of surgery
- Unevenness, lumps or asymmetry, sometimes needing a second session
- A fluid collection (seroma) or infection
- Blood clots in the legs or lungs
- Numbness or changes in the skin where liposuction was done
Every operation carries risk. A surgeon who lists none is not being straight with you. Yours will go through these with you before you consent.

Is a BBL safe?
It carries more risk than most aesthetic operations, and you should be told so plainly. Most of the serious risk comes from fat being placed into or under the buttock muscle. A trained plastic surgeon places it only in the layer under the skin. Ask anyone you consult which layer they inject, and how they make sure of it.
Can I have a BBL if I am slim?
Only if there is enough fat to harvest. If there is not, you will be told so rather than have more taken than is safe.
How long does a BBL last?
Fat that survives the first few months behaves like the rest of your fat. It stays, and it changes if your weight changes.
What about bum injections?
Liquid silicone and other 'hip and bum' injections sold outside hospitals cause serious and lasting harm, and nobody should accept them. If fat transfer is not right for you, the alternatives are discussed honestly at the consultation.
