Signs to look for

Kidney Function Test (Creatinine, eGFR and Urine Protein)

  • Usually none in early kidney disease, which is why testing matters
  • Swollen ankles, feet or hands
  • Passing more or less urine than usual, or foamy urine
  • Blood in the urine
  • Tiredness, poor appetite, itching or breathlessness in advanced disease

Kidney disease usually causes no symptoms until it is advanced, so the only way to find it early is a test. A kidney function test is a blood test that measures creatinine, a waste product the kidneys clear, and uses it to estimate how well the kidneys filter, the eGFR. Nigerian laboratories often report it as electrolytes, urea and creatinine (E/U/Cr). A urine test for protein, ideally the albumin to creatinine ratio (ACR), completes the check, because protein leaking into the urine is an early sign of kidney damage. A first low result is repeated before anyone is told they have kidney disease.

Symptoms

  • Usually none in early kidney disease, which is why testing matters
  • Swollen ankles, feet or hands
  • Passing more or less urine than usual, or foamy urine
  • Blood in the urine
  • Tiredness, poor appetite, itching or breathlessness in advanced disease
  • High blood pressure that is hard to control

When to See a Doctor

Go to a hospital now if you pass very little or no urine for a day, are breathless with swollen legs, become confused or very drowsy, or have been told your potassium is dangerously high. Ask for a kidney function test, blood creatinine with eGFR and a urine test for protein, if you have high blood pressure, diabetes, heart disease, a previous kidney injury, a family history of kidney failure, or have taken painkillers such as ibuprofen or diclofenac regularly. See a kidney specialist if your eGFR is low on repeat tests or falling quickly, or if there is a lot of protein or blood in your urine.

Treatment options

  • A blood test for creatinine, reported with an eGFR
  • A urine test for protein, ideally an albumin to creatinine ratio (ACR)
  • A repeat test within two weeks of a first low eGFR, to rule out a sudden, reversible change
  • Control of blood pressure and blood sugar, which protects the kidneys
  • A review of medicines and herbal remedies that can harm the kidneys
  • Referral to a nephrologist for a low or falling eGFR, or heavy protein in the urine

Common Questions

What does a low eGFR mean?

An eGFR below 60 on tests repeated over three months, or protein in the urine over the same time, is chronic kidney disease. One low result on its own can come from dehydration, a recent illness or a new medicine, which is why it is repeated. A doctor reads the figure with your age, blood pressure and urine result.

Is E/U/Cr the same as a kidney function test?

Mostly, yes. It measures electrolytes, urea and creatinine. Ask for the eGFR to be reported with it, and add a urine test for protein, because a normal creatinine does not rule out early kidney damage.

Can a doctor go through my results by video?

Yes. A doctor can explain your results, check your blood pressure readings and medicines, and arrange repeat tests or a nephrologist's review, all by video.

Need Help?

A doctor on a video call, who advises on the right test and treatment, and tells you if you need to be seen in person.
