Syphilis
The signs, what causes it, and when to see a internal medicine specialist in Lagos.
Signs to look for
Syphilis
- A single painless sore on the genitals, anus or mouth
- Swollen glands in the groin
- A rash on the body, often on the palms of the hands and soles of the feet
- Flu-like illness, patchy hair loss or white patches in the mouth
- Often no symptoms for years
Syphilis is a bacterial infection passed through sex and from a mother to her baby in pregnancy. It begins as a single painless sore that heals on its own, then can return weeks later as a rash, before going quiet for years. Untreated, it can damage the heart, brain and eyes, and in pregnancy it can cause miscarriage, stillbirth or a baby born with the infection. It is found with blood tests and cured with penicillin, best of all when it is caught early.

Symptoms
- A single painless sore on the genitals, anus or mouth
- Swollen glands in the groin
- A rash on the body, often on the palms of the hands and soles of the feet
- Flu-like illness, patchy hair loss or white patches in the mouth
- Often no symptoms for years
When to See a Doctor
Test if you have a sore on or around the genitals, an unexplained rash on your palms or soles, a partner with syphilis, or at the start of every pregnancy. Blood tests can take up to twelve weeks after the risk to turn positive. See a doctor promptly for any new change in vision, hearing or behaviour after a diagnosis, and every pregnant woman with a positive test must be treated the same week.
Treatment options
- A blood test (VDRL or RPR) and a treponemal test to confirm it
- A penicillin injection, one dose for early syphilis and three weekly doses for later stages
- Penicillin only in pregnancy, to protect the baby
- Repeat blood tests over the following year to confirm the cure
- Testing and treatment of partners
- An HIV test, because the two are often found together
Common Questions
Can syphilis be cured?
Yes. Penicillin cures syphilis at every stage, though damage done in the late stages may not reverse, which is why early testing matters.
The sore went away. Am I cured?
No. The sore heals on its own while the infection carries on in the body. Only treatment cures it.
Why is syphilis tested in pregnancy?
Because it passes to the baby and can cause stillbirth or serious illness, and treating the mother early in pregnancy prevents almost all of it.
