Signs to look for

Cold Sores

  • Tingling, itching or burning on the lip before the sore appears
  • Small fluid-filled blisters on or around the lips
  • Blisters that burst, crust over and heal
  • With the first infection in children: fever, sore gums and many mouth sores

Cold sores are small, painful blisters on or around the lips, caused by the herpes simplex virus. After the first infection, often in childhood, the virus stays in the nerves and can flare up again, triggered by illness, fever, sunlight, stress or a period. A tingling or burning feeling often comes a day before the blisters, which burst, crust and heal within about ten days. Cold sores are contagious until they have fully healed. Antiviral cream started at the first tingle can shorten an attack, and tablets help people who get them often.

When to See a Doctor

See a doctor if a cold sore has not healed in ten days, is very large or painful, if you get them often, or if you are pregnant or have a weakened immune system. Go to a hospital the same day if a baby under one month has a cold sore or blisters, if sores spread to the eye or cause eye pain or blurred vision, or if a child with eczema develops spreading painful blisters with fever.

Treatment options

  • Antiviral cream such as aciclovir, started at the first tingle
  • Paracetamol or ibuprofen for pain
  • Cold sore patches to cover and protect the sore
  • Antiviral tablets for frequent or severe attacks, prescribed by a doctor
  • Sunblock lip balm if sunlight triggers them

Common Questions

How do I stop spreading a cold sore?

Do not kiss anyone, especially babies, or share cups, towels or lip balm while you have a sore. Avoid touching it, and wash your hands after applying cream.

Is a cold sore a sexually transmitted infection?

Cold sores are usually caught in childhood through ordinary contact such as kissing, not sex. The same virus can be passed to the genitals through oral sex.

Why are cold sores dangerous for newborns?

A newborn's immune system cannot control the herpes virus, so it can cause a serious infection. Anyone with a cold sore should not kiss a young baby.
