Mastitis and Breastfeeding Pain
The signs, the danger signs that mean hospital now, and how to see a doctor by video from anywhere in Nigeria.
Signs to look for
Mastitis and Breastfeeding Pain
- Sore, cracked or bleeding nipples
- A tender, hard lump in the breast
- A hot, swollen area of breast, red or darker than the surrounding skin
- Pain or burning while feeding
- Fever, chills and aching
Breastfeeding should not hurt once feeding is established. Sore or cracked nipples are usually caused by the baby's latch, and help with positioning often fixes them. Mastitis is inflammation of the breast, usually when milk builds up in part of it; it causes a painful, hot, swollen area and can come with fever and flu-like aches. It often settles within a day or two of feeding or expressing frequently from that side. If it does not, or there is a fever, antibiotics that are safe while breastfeeding are needed. Rarely, a collection of pus called an abscess forms and needs draining.

Symptoms
- Sore, cracked or bleeding nipples
- A tender, hard lump in the breast
- A hot, swollen area of breast, red or darker than the surrounding skin
- Pain or burning while feeding
- Fever, chills and aching
- A painful lump that does not go down after feeding, which can be an abscess
When to See a Doctor
See a doctor if a painful area of the breast has not improved within a day of frequent feeding, if you have a fever, or if nipple pain makes feeding hard. Go to a hospital the same day if you have a high fever, feel very unwell, the redness is spreading fast, or there is a painful swelling that may be an abscess.
Treatment options

- Paracetamol or ibuprofen, which are safe while breastfeeding
- Cold compresses after feeds, and rest
- Antibiotics safe for breastfeeding, when infection is likely
- Drainage of an abscess, usually with a needle
