Nappy rash is sore, irritated skin in the nappy area, caused mainly by urine and stool staying against the skin. On brown and black skin it can look darker rather than red. Many babies get it at some point, and it usually clears within a few days with frequent nappy changes, time without a nappy and a thin layer of barrier cream. A rash that does not clear, that goes into the skin creases, or has small spots spreading out from its edge may be thrush, a yeast infection that needs an antifungal cream. A rash with blisters, crusts or pus may be infected and needs a doctor.

When to See a Doctor
Go to a hospital now if your baby has a rash that does not fade when you press a glass on it, is floppy or very drowsy, or is not feeding, or if a baby under three months has a temperature of 38°C or more. See a doctor if the rash has not improved after a few days of care at home, gets worse or spreads, has blisters, pus or crusts, goes into the skin folds, or if your baby has a fever or seems very uncomfortable. A paediatrician by video can look at the rash on camera and advise on the right cream.
Treatment options
- Changing wet or dirty nappies promptly, and keeping the skin clean and dry
- Time without a nappy whenever you can
- Cleaning with water, or wipes without fragrance or alcohol
- A thin layer of barrier cream at each change, as a pharmacist advises
- No soap, baby lotion, bubble bath, talcum powder or antiseptic creams on the area, and no nappies fastened too tight
- An antifungal cream for thrush, a mild steroid cream for inflamed skin, or an antibiotic for infection, only on a doctor's advice

Common Questions
Should I use powder on nappy rash?
No. The NHS advises against talcum powder in the nappy area. Keep the skin clean and dry, and use a thin layer of barrier cream instead.
Is nappy rash caused by teething?
Teething does not cause nappy rash. If your baby has diarrhoea or a fever as well as a rash, that is an illness to look for, not teething.
Can a paediatrician treat nappy rash by video?
Yes, in most cases. A paediatrician can look at the rash on camera, tell nappy rash from thrush or infection, and advise on the right cream.
Need Help?
A children's doctor on a video call, who tells you whether your child needs to be examined in person.
