An undescended testis is a testicle that has not moved down into the scrotum by birth. It is more common in boys born early. Some come down on their own in the first few months, but one that is still not in the scrotum at about six months is unlikely to move and is brought down with an operation called orchidopexy, ideally by about 12 to 18 months of age. Early treatment protects fertility and makes the testicle easy to check later in life.

Symptoms
- An empty or flat-looking scrotum on one or both sides
- A testicle that can be felt in the groin but not in the scrotum
- A testicle that cannot be felt at all
- A swelling in the groin, which may be a hernia that often comes with it
When to See a Doctor
Ask a doctor to check at birth and again at around three to six months if a testicle is not in the scrotum. See a paediatric surgeon if it has not come down by six months, or if a testicle that was down has moved back up. Go to a hospital immediately if a boy has sudden severe pain or swelling in the groin or scrotum, which can be a twisted testicle and needs surgery within hours.
Treatment options
- Examination and rechecking in the first months of life
- Orchidopexy, usually as a day case, ideally by about 18 months
- Keyhole surgery to find a testicle that cannot be felt
- Repair of a hernia found at the same time
- Teaching self-examination of the testicles from the teenage years

Common Questions
Will an undescended testis come down by itself?
Sometimes, in the first few months of life. After about six months it rarely does, which is why surgery is then advised.
Is it too late if my son is already older?
No. Surgery is still worthwhile at any age. A surgeon will advise on bringing the testicle down or, in some older boys, removing it.
