Most lumps and swellings in the scrotum are not cancer. Common harmless causes are a hydrocele (fluid around the testicle), a varicocele (swollen veins), a cyst on the tube behind the testicle, and a hernia coming down from the groin. A hard, painless lump in the testicle itself is different: it can be testicular cancer, which is most common in men aged about fifteen to forty-five. Testicular cancer is one of the most curable cancers, even when it has spread, so the answer is to have any new lump examined and scanned within days rather than watched.

Symptoms
- A hard, painless lump in or on a testicle
- A testicle that has become bigger, heavier or changed shape
- A dull ache or dragging feeling in the scrotum
- A soft swelling around the testicle (often a hydrocele)
- A lump above the testicle that feels like a bag of worms (often a varicocele)
- In cancer that has spread: back pain, a cough or swollen breasts
When to See a Doctor
Go to an emergency department now for sudden, severe pain in a testicle, especially with feeling sick or tummy pain. A twisted testicle (torsion) needs an operation within hours to save it, and a scan should not delay that. See a doctor within days for any new lump, swelling or change in a testicle, even if it does not hurt. See a doctor promptly for a painful, red, swollen scrotum with fever, which can be an infection.
Treatment options
- An examination and a scrotal ultrasound scan
- Tumour marker blood tests where a tumour is suspected
- Orchidectomy: removal of the testicle through the groin for a tumour
- Hydrocele repair or varicocele repair for those swellings, where they cause trouble
- Antibiotics for an infection of the testicle or the tube behind it
- Chemotherapy or radiotherapy after surgery for some testicular cancers

Common Questions
How do I check my testicles?
Once a month, after a warm bath or shower, roll each testicle gently between your thumb and fingers. Get to know their usual size and feel. The tube behind each one is normal. See a doctor about anything new, hard or different.
Is a painful lump less worrying than a painless one?
Not necessarily. Pain is more often from infection or a cyst, but a tumour can ache too. Any new lump should be examined and scanned, whether or not it hurts.
Will I lose my testicle if it is cancer?
Yes, the affected testicle is removed, which treats the cancer and confirms the diagnosis. Most men have normal sex lives and many father children afterwards with the remaining testicle. Sperm can be frozen first.
