Hand, foot and mouth disease is a common viral infection of young children, especially those under five in creches and nursery schools. It starts with a mild fever and sore throat, then painful ulcers in the mouth and a rash of small spots or blisters on the hands, feet and sometimes the nappy area. It is not related to foot and mouth disease in cattle. It usually clears by itself within seven to ten days. The main risk is a child refusing to drink because the mouth hurts, which can lead to dehydration.

When to See a Doctor

See a doctor if your child is not drinking well, has a fever for more than three days, or the symptoms have not improved after ten days, or if the child is a baby under six months. Go to a hospital now for signs of dehydration (no wet nappy or urine for eight hours or more, a dry mouth, sunken eyes, no tears), or if the child is very drowsy, floppy, has a stiff neck, jerky movements or a fit.

Treatment options

  • Plenty of cool drinks in small, frequent sips
  • Soft, cool foods such as yoghurt, pap or mashed foods
  • Paracetamol or ibuprofen for pain and fever
  • Avoiding salty, spicy and acidic foods while the mouth is sore
  • Handwashing and keeping the child home while they feel unwell

Common Questions

Does my child need antibiotics?

No. It is caused by a virus, so antibiotics do not help. Pain relief and keeping your child drinking are what matter while it passes.

When can my child go back to creche?

Once they feel well enough, usually when the fever has gone and they are eating and drinking. The virus can stay in the stool for weeks, so careful handwashing after nappy changes matters.

Can adults catch it?

Yes, though it is usually milder. Wash your hands after changing nappies and helping your child, and avoid sharing cups.

Need Help?

A children's doctor on a video call, who tells you whether your child needs to be examined in person.
