Oral thrush is a common yeast infection in babies' mouths, caused by candida. It shows as white patches on the tongue, gums, inside the cheeks or the roof of the mouth that look like milk curds but do not wipe away easily. It is common in the first months of life, and more likely after antibiotics. It is usually mild and clears with an antifungal gel or drops from a doctor. A breastfeeding mother can develop thrush on the nipples too, so mother and baby may need treatment together to stop it passing back and forth.

Symptoms

  • White patches on the tongue, gums or cheeks that do not wipe off
  • Red, raw areas if the patches are rubbed
  • Fussing or pulling off the breast or bottle during feeds

  • In the mother: sore, cracked or shiny nipples, or shooting pain while feeding

When to See a Doctor

See a doctor if you see white patches that do not wipe away, if your baby is feeding less, or if thrush keeps coming back. Go to a hospital if your baby refuses all feeds, has fewer wet nappies, a fever, or is very drowsy or floppy.

Treatment options

  • An antifungal gel or drops, prescribed by a doctor
  • Treating the mother's nipples at the same time if breastfeeding
  • Sterilising bottles, teats and dummies after each use
  • Washing hands before and after feeds
  • Treating a thrush nappy rash with an antifungal cream

Common Questions

Is it milk or thrush?

Milk wipes away easily with a soft cloth. Thrush patches stay put, or leave a red, sore patch underneath when rubbed.

Can I keep breastfeeding?

Yes. Keep breastfeeding, and ask a doctor to treat your nipples at the same time as your baby, so thrush does not pass between you.

Why does it keep coming back?

Usually because bottles, teats and dummies, or the mother's nipples, are reinfecting the baby. Treating both and sterilising everything that goes in the mouth stops the cycle.
