Blood in the urine, called haematuria, can turn urine pink, red or the colour of cola, or be found only on a urine test. It is rarely an emergency, but it always needs a cause found. Common causes are a urine infection, a kidney or bladder stone and an enlarged prostate. In parts of Nigeria, bilharzia (schistosomiasis) from swimming or washing in fresh water is a common cause in young people. In adults, especially smokers and anyone over about forty, blood in the urine can be the first sign of a cancer of the bladder or kidney, and it often stops on its own even when the cause is still there.

Symptoms
- Pink, red or brown urine
- Clots or strings of blood in the urine
- Blood seen only at the end of passing urine
- Burning or pain when passing urine, with an infection
- Pain in the side or back, with a stone
- Often no pain or other symptoms at all
When to See a Doctor
Go to an emergency department now if you cannot pass urine because of clots, if the bleeding is heavy, if you have fever and shivering with pain in the side or back, or if the blood follows an injury. Otherwise, see a doctor within days for any blood you can see in your urine, even if it happened once and has stopped. Some foods and medicines change the colour of urine, and a simple urine test tells the difference.
Treatment options
- A urine test to confirm blood and check for infection
- Blood tests for kidney function
- An ultrasound or CT scan of the kidneys and bladder
- Cystoscopy, a camera look inside the bladder, for adults with visible blood
- Antibiotics for an infection, and treatment for bilharzia where it is the cause
- Treatment of the cause once found: a stone, the prostate or a tumour

Common Questions
The blood stopped. Do I still need to be checked?
Yes. Bleeding from a bladder or kidney tumour often comes and goes, so stopping does not mean the cause has gone. A urine test, a scan and, for most adults, a cystoscopy are how it is ruled out.
Can food or medicine make urine look red?
Yes. Beetroot, some food colourings and some medicines, such as the TB medicine rifampicin, can change the colour. A urine test shows whether there is real blood.
Is blood in the urine always serious?
No. Most cases have a simple cause, such as an infection or a stone. It is checked because the serious causes are the ones that cause no pain, and they are easier to treat when found early.
Can bilharzia cause blood in the urine?
Yes. In parts of Nigeria, bilharzia picked up from rivers, ponds and streams is a common cause of blood at the end of passing urine, especially in children and young people. It is treated with tablets, and long-standing untreated bilharzia can damage the bladder.
