Hypospadias is a condition present at birth in which the opening of the urine tube is on the underside of the penis rather than at the tip. The foreskin is often incomplete, giving a hooded look, and the penis may bend. It is usually repaired by a paediatric surgeon between about six and eighteen months of age. Because the foreskin is often used in the repair, a baby boy with hypospadias should not be circumcised until a surgeon has seen him.

Symptoms
- The urine opening on the underside of the penis, or near the scrotum
- A hooded foreskin that does not cover the front of the penis
- A downward bend of the penis, often seen on erection

When to See a Doctor
Ask a doctor to examine any baby boy before circumcision, and see a paediatric surgeon if hypospadias is found. Ask for an assessment before six months so the repair can be planned. See a doctor urgently if a boy cannot pass urine, or has pain, swelling or a fever after a repair.
Treatment options
- Examination by a paediatric surgeon, including both testicles
- Delaying circumcision until the surgeon has advised
- Single-stage repair for most openings near the tip
- Staged repair for openings near the scrotum
- Follow-up of the urine stream once the boy is toilet trained

Common Questions
Can my son be circumcised?
Not before a surgeon has seen him. The foreskin is often used to rebuild the urine tube. Circumcision can usually be done as part of the repair.
Will he be normal as an adult?
After a successful repair, most boys pass urine standing and have normal function as adults. Some need a second operation for a leak or narrowing.
