Diabetic Retinopathy
The signs, what causes it, and when to see a ophthalmology specialist in Lagos.
Signs to look for
Diabetic Retinopathy
- No symptoms at all in the early stages
- Blurred or fluctuating vision
- Difficulty reading or seeing fine detail
- Dark spots or new floaters in the vision
- Colours looking washed out
Diabetic retinopathy is damage to the small blood vessels of the retina, the seeing layer at the back of the eye, caused by high blood sugar over time. Vessels leak, close off, or grow fragile new branches that can bleed. When fluid collects at the centre of vision it is called diabetic macular oedema. With diabetes so common in Nigeria and so often found late, many people learn they have retinopathy only when their sight has already fallen. In its early stages it causes no symptoms at all, which is why a dilated eye check every year matters even when vision feels normal.

Symptoms
- No symptoms at all in the early stages
- Blurred or fluctuating vision
- Difficulty reading or seeing fine detail
- Dark spots or new floaters in the vision
- Colours looking washed out
- Sudden loss of vision if a vessel bleeds
When to See a Doctor
Everyone with diabetes should have a dilated eye examination when diabetes is diagnosed and then at least once a year, more often in pregnancy or once retinopathy is found. See an eye specialist promptly if your vision blurs or changes. Go the same day for sudden loss of vision, a shower of floaters, or a shadow across your sight.
Treatment options
- Good control of blood sugar, blood pressure and cholesterol, which slows the damage
- Regular dilated eye checks and retinal photographs
- Laser photocoagulation for fragile new vessels
- Anti-VEGF injections for swelling at the centre of vision
- Vitrectomy surgery for bleeding inside the eye or a pulling scar on the retina

Common Questions
My sight is fine. Do I still need an eye check?
Yes. Diabetic retinopathy usually causes no symptoms until it is advanced. A dilated check finds it while treatment can still protect your sight.
Can diabetic retinopathy be reversed?
Damage that has already taken sight usually cannot be undone. Treatment stops it getting worse, and swelling at the centre of vision often improves with injections. Better control of sugar and blood pressure slows it.
Does type 2 diabetes affect the eyes as much as type 1?
Yes. Both types cause retinopathy. Because type 2 is often present for years before diagnosis, some people already have eye damage when they are first told they have diabetes.
Should I use herbal eye drops for blurred vision from diabetes?
No. Herbs and unprescribed drops do not treat retinopathy and can infect the eye. Blurring in diabetes needs a dilated examination to find the cause.
