The whole prostate and the seminal vesicles are removed, sometimes with nearby lymph glands, to cure prostate cancer that is still inside the gland. It is done open through a cut in the lower tummy, or by keyhole surgery. Radiotherapy is the main alternative, and for small, slow-growing cancers careful monitoring may be the better choice. You should hear about all three before you decide.

Is it right for you

Who this is for

A good fit

  • Prostate cancer confirmed by biopsy that scans show has not spread beyond the prostate
  • A cancer graded as intermediate or high risk, where monitoring alone is not safe
  • A man fit for a major operation with many years of life ahead of him

When it is not the right answer

  • Small, low-grade cancers, which can often be watched safely with PSA tests and MRI
  • Cancer that has spread to the bones or lymph glands, which is treated with hormone therapy and other medicines
  • A man with serious heart or lung disease, for whom radiotherapy may be safer
  • An enlarged prostate without cancer, which is treated with tablets or TURP

What happens

The pathway, step by step

  1. 1

    Consultation with the urologist₦50,000

Your PSA, biopsy grade and scans reviewed, and the choices between surgery, radiotherapy and monitoring explained.

  2. 2

    Staging tests

MRI of the prostate and, where the cancer is higher risk, a bone scan or CT, to confirm it has not spread.

  3. 3

    A written price

The operation and the stay, in writing, before admission.

  4. 4

    The operation

Two to four hours under general anaesthetic.

  5. 5

    Hospital stay

Usually two to five nights. You go home with a catheter.

  6. 6

    Catheter out and results₦25,000

The catheter comes out after one to two weeks. The tissue result and PSA tests decide whether anything more is needed.

Afterwards

Recovery, realistically

First two weeks

At home with a catheter. Walking every day, no heavy lifting.

After the catheter is out

Leaking urine is common at first. Pelvic floor exercises, started early, help most men regain control over weeks to months.

Weeks 6 to 12

Back to most normal activity. Your first PSA test after surgery should be very low.

Months 3 to 24

Erections can take a year or more to recover, if they do. Treatments are available to help.

Risks you should be told about

  • Leaking urine, usually improving over months, sometimes lasting
  • Erection problems, depending on age, erections before surgery and whether the nerves could be spared
  • No semen at orgasm and loss of natural fertility, which are permanent
  • Bleeding needing a transfusion
  • Infection and blood clots
  • Narrowing where the bladder is joined to the urethra

Every operation carries risk. A surgeon who lists none is not being straight with you. Yours will go through these with you before you consent.

Questions people ask

Surgery or radiotherapy: which is better?

For cancer inside the prostate, both can cure it and neither is right for every man. They differ in their side effects and in what is possible afterwards. A good urologist will explain both and, where possible, let you speak to a cancer doctor who gives radiotherapy.

Will I be able to have sex afterwards?

Orgasm is still possible, but no semen comes out. Erections are often weaker or absent for a while, and whether they recover depends on your age, how good they were before and whether the nerves could be spared. Medicines and other treatments can help.

Will I leak urine?

Most men leak at first, and most regain good control over the following months, helped by pelvic floor exercises. A small number have leaking that lasts, and there are treatments for that too.

Why is there no fixed price on this page?

Because the approach, the stay and whether lymph glands are removed all change it. You get the price in writing before admission.
