A urethral stricture is a scarred, narrowed section of the tube that carries urine out of the body. A short, first-time stricture can be cut open from inside with a telescope (urethrotomy). A stricture that is long or has come back after stretching or urethrotomy is repaired by urethroplasty: the scarred section is removed and the ends joined, or the tube is widened with a graft, often of lining taken from inside the cheek. Urethroplasty is the repair most likely to last.

The operation
Urethrotomy takes under an hour through a telescope. Urethroplasty takes two to four hours under general or spinal anaesthetic.

A catheter rests the repair: days after urethrotomy, usually two to three weeks after urethroplasty.

An X-ray checks the repair has healed before the catheter comes out. Flow tests continue for a year or more.

Recovery, realistically
First days
Sore where the cut was made. If a cheek graft was taken, the mouth is sore and soft food helps for a week.
With the catheter
Keep it clean and the bag below the bladder. Desk work is often possible.
After the catheter is out
The stream is usually much stronger. Some spraying or dribbling after finishing can continue.
The first year
Regular flow checks, because a stricture that is going to come back usually shows itself within this time.
Risks you should be told about
- The stricture coming back, more likely after urethrotomy than urethroplasty
- Urine infection
- Bleeding or bruising
- Dribbling after passing urine
- Numbness or tightness in the cheek where a graft was taken
- Changes to erections or ejaculation, uncommon

Questions people ask
Will stretching the urethra cure my stricture?
Stretching and urethrotomy can help a short stricture, but a stricture that has come back once is likely to keep coming back with them. Urethroplasty repairs it, which is why it is offered to men who have already had one of the quicker treatments.
What causes a urethral stricture?
Scarring. In Nigeria common causes are past gonorrhoea or other infections that were not fully treated, injuries to the groin or pelvis, and earlier catheters or operations. Sometimes no cause is found.
Why is there no fixed price on this page?
Because a short urethrotomy and a long graft repair are very different operations. The urethrogram decides which you need, and you get the price in writing before admission.
