A tooth is removed when it cannot be saved: badly decayed, broken below the gum, loose from gum disease, or a wisdom tooth causing repeated trouble. A simple extraction is done under local anaesthetic in a few minutes. A surgical extraction, for a tooth that is broken or still partly under the gum, takes longer and needs a small cut and stitches. You are told which you need, and the price, before anything is done.

Also called: tooth removal, wisdom tooth removal, surgical extraction, removing a tooth

Who this is for

  • A tooth too decayed or broken to be filled or crowned
  • A wisdom tooth that keeps causing pain, swelling or infection of the gum over it
  • A tooth loosened by advanced gum disease
  • A tooth the dentist advises removing before braces or a denture

What happens, step by step

  1. 1

    Examination and X-ray

The dentist checks whether the tooth can be saved and takes an X-ray to see its roots. You are told the options, including keeping the tooth, and the price of each.

  2. 2

    Numbing

A local anaesthetic numbs the tooth and the gum around it.

  3. 3

    Removal

The tooth is loosened and lifted out. For a surgical extraction a small flap of gum is opened and the tooth may be divided, then the gum is stitched.

  4. 4

    Aftercare and follow-up

You are told how to look after the socket, given a number to call, and booked back if stitches need removing.

Afterwards

The first 24 hours

Bite on gauze to stop the bleeding. Do not rinse, spit hard, smoke or drink through a straw, so the blood clot stays in the socket.

Days two to seven

Gentle warm salt-water rinses after meals. Soreness and some swelling are normal and should improve each day.

If it gets worse

Pain that gets worse after two or three days can be a dry socket. Call us; it is treated with a dressing in the socket.

Risks you should be told about

  • Bleeding, swelling and soreness for a few days
  • Dry socket, when the clot is lost and the socket becomes very painful
  • Infection of the socket
  • With lower wisdom teeth, rarely, numbness of the lip or tongue from bruising of a nerve, usually temporary

The dentist goes through the risks for your own mouth before you agree to treatment, and you can always say no.

Questions people ask

Does tooth extraction hurt?

The tooth is numbed, so you feel pressure but should not feel pain while it is removed. It is sore afterwards for a few days, and simple painkillers usually help.

Can a swollen tooth be removed?

Sometimes the infection has to be treated first. If your face or neck is swelling, or you have a fever or difficulty swallowing, go to a hospital emergency department now.

Should I replace the tooth?

Not always. A gap at the back may cause no trouble. A gap at the front, or several gaps, can be filled with a denture, a bridge or an implant, and the dentist explains each.

How soon can I eat?

Once the numbness wears off. Eat soft food on the other side for the first day or two and avoid very hot drinks.
